Please read and follow these steps carefully to be fully registered.
All Athletes must have the following forms on file with the Saydel Activities Office before they will be allowed to participate.

Form 1: Athletes must have a current sports qualifying physical exam on file. Physicals are good for 13 months from exact date of last physical. Download a physical form at the Iowa High School Athletic Association site HERE.
Once you have your physical completed by a doctor, you can turn it in to the office or you can scan it and submit the completed Physical Form online. |

| Form 2: Athletes must have a current emergency card, medical waiver, and concussion form on file. These forms are good for 12 months from the date it is submitted online. You can access these forms above or by clicking HERE. |

Other forms may be required based on different sports/events. These forms can be found on the individual sport's pages or by contacting the coaches involved.
